PARTICIPANT WAIVER, RELEASE OF LIABILITY & MEDIA CONSENT

Coast-to-Coast Connection – Lake Eola Community Event

PLEASE READ CAREFULLY BEFORE SIGNING

I understand that participation in the Coast-to-Coast Connection Lake Eola event (the “Event”) may include physical and recreational activities such as walking, running, stretching, yoga, Tai Chi, dance, rhythmic movement, drumming, demonstrations, workshops, and other group activities. I voluntarily choose to participate and understand that these activities involve inherent risks, including but not limited to falls, collisions, overexertion, muscle or joint injury, dehydration, heat-related illness, adverse weather, uneven surfaces, equipment-related injury, contact with other participants, and other known or unforeseen risks that may result in injury, illness, property damage, disability, or death.

ASSUMPTION OF RISK & RELEASE OF LIABILITY

I knowingly and voluntarily assume all risks associated with my participation in the Event. To the fullest extent permitted by Florida law, I release and hold harmless Miles with Purpose; World Youth Clubs; Orlando Ballet and its Emotions in Motion program; Peaceful Peacock Yoga; the City of Orlando; and all other participating organizations, event partners, instructors, facilitators, performers, sponsors, vendors, volunteers, contractors, property owners, venue operators, officers, directors, employees, agents, representatives, affiliates, successors, and assigns (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, expenses, or liabilities arising from or related to my participation in or attendance at the Event, including claims resulting from the ordinary negligence of a Released Party, except to the extent such liability cannot legally be waived.

I acknowledge that I am responsible for determining whether I am physically capable of participating in any activity and agree to stop participating if I feel unsafe, ill, injured, or unable to continue.

MEDICAL TREATMENT

If I require emergency medical care during the Event, I authorize Event personnel to seek reasonable emergency assistance on my behalf. I understand that the Released Parties do not assume responsibility for medical expenses incurred as a result of my participation.

PHOTO, VIDEO & MEDIA RELEASE

I understand that photography, video, audio recording, and other media may be created during the Event. I grant Miles with Purpose and participating Event partners permission to photograph, film, record, and use my name, image, likeness, voice, and appearance in connection with the Event. I authorize such materials to be used, reproduced, edited, published, displayed, and distributed for social media, websites, press and media coverage, educational materials, fundraising, advertising, publicity, and other promotional purposes, in any media now known or later developed, without compensation to me. I waive any right to inspect or approve the finished materials or their use.

ACKNOWLEDGMENT

I have read this waiver, understand its terms, and understand that by signing it I am giving up certain legal rights. I sign voluntarily and agree that this waiver is intended to be as broad and inclusive as permitted by Florida law.

NOTICE TO THE MINOR CHILD’S NATURAL GUARDIAN

**READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF MILES WITH PURPOSE, WORLD YOUTH CLUBS, ORLANDO BALLET/EMOTIONS IN MOTION, PEACEFUL PEACOCK YOGA, THE CITY OF ORLANDO, AND THE OTHER RELEASED PARTIES IDENTIFIED BELOW USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY

PARENT OR LEGAL GUARDIAN CONSENT AND SIGNATURE

I certify that I am the parent or legal guardian of the minor participant named above, or that I am legally authorized to provide consent on the minor’s behalf. I have carefully read and understand this Waiver, Release of Liability, Assumption of Risk, and Media Consent. I voluntarily consent to the minor’s participation in the Event and agree to the terms of this agreement to the fullest extent permitted by Florida law.
